Please confirm each of the following. All items must be checked before the sauna is handed over.

I confirm that the site Risk Assessment has been carried out by Saddle Up Sauna at delivery, and I have been made aware of and understand the relevant risks and control measures identified.

I confirm that I understand the location and use of the fire extinguisher and fire blanket.

I confirm that I understand the emergency contact procedures.

I confirm that a safe location has been agreed for fuel storage on the site.

I confirm that I have received the Sauna Operating Manual.

I confirm that I have received and understand the Health & Safety Waiver sent by email and acknowledge this on behalf of all users at the site.

I confirm that I have received and understand the Terms & Conditions of Use sent by email.
